Disease Management

The MRG disease management program stresses prevention and education to promote lifestyle change, improve medical self-management, and to increase member and family satisfaction.

The key intent of disease management is to improve health outcomes for individuals living with chronic diseases and eliminate medical crisis' to the greatest possible degree. Disease management processes include risk assessments and interventions designed to improve member compliance with recommended interventions that prevent acute events. Baseline assessments provide a screening process that identifies the members' chronic disease history and provides a stratification of members into severity levels. Interventions include implementation of standard clinical guidelines for recommended disease specific treatments established for the members' severity level. These interventions also include member and provider education focusing on member self-management.
